Maryland wins $2.4M federal grant for medical home program
Maryland will be getting nearly $2.4 million from the Centers for Medicare and Medicaid Services to put toward expanding its patient centered medical home program.
The award is part of nearly $300 million in State Innovation Model Initiative grants, which are intended to fund state-wide programs aimed at reforming the health care delivery system, reducing costs and improving quality. State programs are expected to focus on Medicare Medicaid and the Children’s Health Insurance Program.
